Attendance | Cancellation | Lateness Policy
We kindly request that you give your therapists 24 hours notice of cancellation in order to accommodate fellow patients and avoid inconvenience to all. We also ask that you arrive at your scheduled appointment times unless otherwise arranged. This will ensure that you and your fellow patients receive the proper care and attention you deserve. You are asked to attend all visits as prescribed. Failure to do so will result in slow recovery, insurance denial, discharge from therapy and will negatively affect your legal cases. Often times, insurance adjusters and case managers will call or visit unexpectedly to verify your attendance. You are expected to attend all visits as prescribed.
Insurance
If you are a motor vehicle (MVA) or personal injury (PIP) patient, you will most likely require pre-authorization from your insurance company prior to initiating PT. We participate in several health plans, including Medicare. As a courtesy to you, we will gladly verify your insurance benefits. However, please understand that verification of coverage is ultimately your responsibility. You can call the telephone number on your insurance card and ask for a copy of your in AND out of network benefits (usually one to two pages). Insurance companies are happy to fax you or us (877.9NJ.ABLE/965.2253) a copy. We ask that you give us a call if you have any questions regarding your insurance.
What to Bring
We request that you bring: • a valid ID • all insurance cards and documentation • attorney letters • medical reports • Xray/MRI/CT results (films are not necessary) • a doctor's prescription.
Also please bring with you a list of your current medications and knowledge of your past medical/surgical history.
If you can download the required forms and complete them prior to your visit, it will allow us to see you sooner.
